You are invited to participate in a study about provision of aftercare to patients who travel a long distance (100 miles or more) to be hospitalized at psychiatric hospitals. The purpose of this study is to determine the aftercare options of people with dissociative disorders, especially those who get their inpatient psychiatric hospital care in specialty hospitals far from home. Your experience and expertise can make an impact on this important and growing concern affecting aftercare and support for patients with dissociative disorders and their significant others. The study is being conducted by Jackie Brodsky, a doctoral student in the College of Communication and Information Sciences at The University of Alabama. I am working under the guidance of Laurie Bonnici, Ph.D., who is a professor in the College of Communication and Information Sciences. We appreciate you taking 20-30 minutes of your time to take part in this important work by completing a questionnaire. We ask that you complete the questionnaire no later than November 30, 2011.
The purpose of this study is to determine the aftercare and support services provided by psychiatric hospitals to people with dissociative disorders and their families.
If you agree to be in this study, we would ask you to complete the questionnaire that follows. You will be asked several general demographic questions as well as questions about the aftercare, family support, and coordination of care that your hospital provides to patients who travel a long distance (100 miles or more) to be hospitalized. Please answer the questions honestly and as completely as possible.
The study has minimal risks. The benefits to participation are involvement in provision of increased understanding of the aftercare services provided to patients who travel a long distance to be hospitalized, as well as support for their families.
No compensation is offered for your participation in this study.
The records of this study will be kept private and confidential to the extent permitted by law. In any sort of report we might publish, we will not include any identifying information. Research records will be stored securely, and only the researcher will have access to the records.
Participation in this study is voluntary. Your decision whether or not to participate will not affect your current or future relations with the university associated with this study. If you decide to participate, you are free to not answer any question or withdraw at any time without affecting those relationships.

Participants will be completing a confidential web-based questionnaire and, after reading the questionnaire invitation, will provide consent by clicking the link to enter the questionnaire.
