This form can be filled out using either Acrobat Reader, the free program, or Adobe Acrobat. The file can be printed out from either program, but can only be saved using Adobe Acrobat.
You can also print out the blank form as before to write in orders by hand. There will be no observable difference.
1) To fill in data field, move cursor over fields until an I-beam cursor is seen. Type in desired text.
2) Print file as usual.
3) Unless you save the form after typing text (only possible with Adobe Acrobat), you can reuse the file as necessary.
