Re:       Protocol for Reporting Biological Incidents       

All emergency incidents must be reported immediately to the laboratory PI or supervisor and EHS. Such incidents include but are not limited to:  missing materials, biologicals or chemicals; inadvertent fires; explosions; personnel exposures; injuries; releases of biohazard materials and failure of biohazard containment. EHS will in turn make (using such help as necessary by the fire authority, medical personnel, Laboratory Biological Safety Officer (LBSO), etc.) such investigations and reports as required. All external reports, other than those of an immediate nature such as summoning the fire department in case of a fire, are to be made by or through EHS.
