UNDERAGE USER AUTHORIZATION APPLICATION
The University of Alabama uses radioactive isotopes and radiation producing machines under a general license from the Bureau of Radiological Health, State of Alabama. As a function of this license the Office of Environmental Health and safety (EHS) administers the Radiation Safety Program (RSP). The objectives of this program are to ensure the responsible use of radiation, to reduce University liability, to ensure University compliance with applicable regulations and to promote the As Low As Reasonably Achievable (ALARA) concept for all exposures.
As a user of radioactive isotopes or radiation producing machines who is under 18 years of age specific permission is required from the University Radiation Safety Officer (RSO). As a user you will receive the same instructions as any University user including detailed information for females. As a trained user you will be responsible for following safety procedures, guidelines, and instructions regarding your work. Failure to follow prescribed policy may result in expulsion from the lab or work area.
Completion of this underage user authorization application is the initial step toward attaining approval to work with isotopes or radiation producing machines. The prospective user must complete Section II and return it to their University supervisor. Following review, approval will be granted or refused.
