EVALUATION FORM
University of Alabama Montgomery Experience Program
Interim, 2011.

Thank you for your role in supervising one of our Montgomery Experience Interns.  Please answer the following questions, and be sure to assign your intern an overall grade (on a 0-100 scale) at the bottom of the third page.   This overall grade will make up 10% of the intern’s grade for the Montgomery Experience course, so consider it carefully.*   Feel free to attach or send samples of your intern’s work, or feedback from others about the work,  if such is available and you think it would assist in the evaluation;  you are not required to do so.

Please e-mail your responses to sborrell@bama.ua.edu.   If you feel more comfortable sending them by regular mail, send them to Steve Borrelli, Dept. of Political Science, PO Box 870213, University of Alabama, Tuscaloosa AL 35487.  I would like to receive the evaluations no later than June 15, 2011, but a late evaluation is better than none at all!

1.)  Please describe, in specific terms, the tasks and/or projects on which your intern worked during his or her three weeks in Montgomery.

2.)  Please estimate the number of hours per week that your intern worked, on average.

3.)  Please describe and evaluate the QUALITY of the work performed by your assigned intern.  (Effort will be evaluated separately below)

4.)  Please describe and evaluate the DEGREE OF EFFORT put forth by your intern in completing his or her assigned tasks.

5.)  Please discuss specific strong points, qualities, and/or skills displayed by your assigned interns.

6.)  Please discuss any areas in which you think your intern  might need work, or could have improved on his or her performance.

7.)   Is there anything we can do on our end, at the University, to better prepare our students for working at your office?  (You can suggest ANYTHING:   academic preparation, communications skills, interpersonal skills, knowledge of Alabama government, etc.)

8.)   Would you recommend any changes in our program itself?  (Again, suggest ANYTHING that comes to mind:  length of program, more academic supervision, better orientation, more guidance for employers, etc.)

9.)  (OPTIONAL).  Would you be interested in participating in our program next year?  Do you know of any other agencies or organizations that did not participate this year, who might be interested in hosting an intern next year?

10.)   PLEASE ASSIGN YOUR INTERN AN OVERALL NUMERICAL GRADE ON A 0-100 STANDARD ACADEMIC SCALE, WHERE 90 AND ABOVE IS AN “A,  80-89 IS A “B,”, 70-79 IS A “C”, 60-69 IS A “D,” AND BELOW 60 IS AN “F.”  If you feel you need to say anything to justify or explain your grade, beyond what you have written above, feel free to do so.
