Alabama laws require certain immunization (protection) against diseases and infections for children from kindergarten through twelfth grade.  Certificates of immunization (sometimes called Blue Slips) are available from local physicians or local health departments.

You can obtain the required form from the County Health Department at 1101 Jackson Avenue.  (That is located off Bear Bryant Ave. between U of A Law School and DCH Hospital.)  Call them at 345-4131 for information.

Normally the certificate is valid for several years.  The physician can advise you.
